The film delves deeper into the series' mythology, exploring the origins of Death's design. The story follows college student Stefanie, played by Kaitlyn Santa Juana, who experiences violent recurring nightmares. In order to save her family from a gruesome fate, she returns home to uncover the truth behind a multigenerational curse.

The cast of the film includes Kaitlyn Santa Juana, Teo Briones, Richard Harmon, Anna Lore, Brec Bassinger, and Tony Todd. This will be Tony Todd's last film appearance.
